Paperwork requests must be handled directly by your provider. If you need a form or letter completed, the best way to start is by messaging your provider through your client portal. If you need a copy of your health records, our Records Team can help.
Why some paperwork requests can't be completed
Not all paperwork requests can be fulfilled, as this process depends on several factors, including:
Licensing and qualifications: State laws may restrict which providers or licenses are allowed to complete certain paperwork. For example, some states limit who can complete Emotional Support Animal (ESA) letters or disability forms.
Insurance requirements: Some insurance plans prohibit providers from completing specific paperwork. For example, Kaiser does not allow providers to complete leave-of-absence letters.
Provider policies and fees: Provider set their own policies for handling paperwork requests. Not all providers can—or are willing to—fulfill every request. Since letter writing and form completion are not billable services through Grow Therapy, providers may charge an out-of-pocket fee.
Please note: Charging separate cash fees for letter writing is permitted, except for individuals covered by Medicaid or Kaiser. Providers cannot charge these fees to clients with this coverage.
