Family and couples therapy on Grow uses the same booking and telehealth process as individual sessions, but with distinct billing and a few platform differences worth knowing. This article covers how sessions are organized, how to bill using the correct CPT codes, and what clients experience on their end.

Indicate that you offer couples therapy
Adding couples therapy as a speciality to your Grow provider profile helps clients who are searching specifically for this service find you. It also sets accurate expectations before a client books.
To indicate that you work with families and couples:
Select Profile from the sidebar
Click here or select the Clients you serve tab.
From the “Clients you work best with” drop-down menu, select Couples Counseling or under “Therapy Type.”
Select Save and view profile or Save at the bottom to ensure your updates are reflected in your Grow Therapy provider profile.
💡 Note: Even if your profile does not list couples counseling as a specialty, clients may still book with the intention of attending as a couple. Clients may also assume that specialties like "family conflict" include couples counseling. See What to do if a couple books with you unexpectedly below.
How couples therapy is structured
One client of record
Grow's platform is structured around one identified client per chart. When you see a couple, one partner is designated as the identified client — typically the person whose insurance is being billed. All session records, notes, and forms are stored under that client's chart.
The other partner is not a separately registered Grow client unless they have their own intake, chart, and billable diagnosis. In that case, they would be a fully separate client in your panel.
Intake forms and paperwork
Only the identified client receives Grow's intake forms before the first session. The partner does not need a Grow account or to complete a separate intake.
That said, you are welcome to send the partner your own consent documentation or a collateral agreement outside of Grow's intake process. Only the identified client is required to acknowledge Grow's Informed Consent.
Viewing client information during a session
During a couple's session, the client sidebar in Grow telehealth displays information for the identified client only — the partner's information will not appear there, since only the identified client has a Grow account. Telehealth logistics for couples sessions
Sharing the session link
Only one Grow telehealth link is generated per appointment. To include the partner in a telehealth session, you or the identified client shares the session link with them in advance. Both individuals click the same link, enter their names when prompted, and wait in the waiting room until you admit them.
To locate a client’s unique Grow telehealth link:
In your Provider Portal, navigate to Appointments via the sidebar.
Search and select the appointment. Both the calendar and list views of your appointments will work.
Under "Location", you’ll see the secure telehealth link (formatted as telehealth.growtherapy.com/[unique-link]) for both you and your client.

Select the copy icon to copy the link and paste it into a message to your client.
This action is also available through the appointment preview on your Grow calendar.
Joining from separate locations
Both partners can attend a session using different devices and locations — they just need to use the same telehealth link. If both clients are in the same location, each person should ideally join from their own device in a private area to ensure good audio quality and privacy.
🛠️ Mobile app note: At this time, the Grow Therapy iOS mobile app does not support multiple participants joining the same session — even when using the same account on two separate mobile devices. For couples sessions, the partner should join via a browser using the shared telehealth link rather than through the app.
Cross-state licensing
If partners are in different states at the time of the session, you must be licensed in both states to conduct the session. This is consistent with standard telehealth licensing requirements and applies to couples sessions just as it does to individual sessions.
Billing for couples sessions
The identified client
When billing for couples or family therapy, you will identify one person as the identified client. This is the person whose insurance is being billed. The CPT codes used for couples sessions are family therapy codes, and they are billed under the identified client's chart — the same way individual sessions are billed under a single client.
CPT codes for couples and family therapy
There are two codes for family and couples therapy:
CPT 90847 — Family or couples therapy with the identified client present. Use this when the identified client is in the session alongside their partner.
CPT 90846 — Family or couples therapy without the identified client present. Use this when you hold a session with the partner alone, as part of the treatment of the identified client. In this scenario, the partner is not registered as a Grow client.
For either code, the session must last at least 26 minutes. If the partner is only part of a session for fewer than 26 minutes, bill using the appropriate time-based individual CPT code for that portion.
⚠️ Important: Grow's platform cannot accommodate more than one session per day for each client, because insurance companies only reimburse once per day per client. When using CPT codes 90846 and 90847 for the same client, these codes must be applied on separate days.
Billing when you see partners individually as part of couples treatment
If you hold individual sessions with each member of a couple as part of their treatment:
Use 90847 or 90846 for sessions that are part of the couples treatment (see above).
Use the appropriate individual CPT code (e.g., 90837, 90834) when seeing the identified client in a standalone individual session.
Offering individual therapy to each partner separately
From a billing standpoint, you may offer individual therapy to each member of a couple in addition to couples sessions. If a partner will be seen for their own individual therapy — not as part of the couples treatment — they must be registered as a separate client in Grow with their own intake and a billable diagnosis.
What to do if a couple books with you unexpectedly
At this time, clients cannot select a session type — such as couples counseling or family therapy — when booking through your Grow booking page. A couple may book what appears to be an individual session, or may assume that a specialty like "family conflict" includes couples work. Because of this, you may occasionally receive a booking where the clients intend to attend as a couple without this being apparent upfront.
If you offer couples therapy
If you offer couples therapy and a couple has booked, you can proceed with the session. Keep in mind:
Confirm which partner will be the identified client for billing purposes.
Share the telehealth link with both partners ahead of the session if it is a telehealth appointment.
Only the identified client needs to complete the intake paperwork.
If you do not offer couples therapy:
If you do not offer couples therapy, there is currently no system-level way to prevent a couple from booking with you. To reduce the likelihood of this happening, we recommend adding a clear note to the Clients you serve section of your profile stating that you do not currently offer couples or family sessions. This makes your scope of practice visible to prospective clients before they book.
If a couple has already booked and you are unable to see them, use the in-portal referral process and select Couples Counseling from the "Client need" drop-down menu to connect them with a provider who offers this service. For steps, see Refer a client to another Grow Therapy provider.
Frequently Asked Questions FAQs
Does the partner need their own Grow account?
No. Only the identified client — the person whose insurance is being billed — needs a Grow account. The partner joins via the telehealth link and does not complete a separate Grow intake.
Does the partner need to fill out intake paperwork?
Grow's intake forms are only required for the identified client. You may choose to send the partner your own consent or collateral documentation outside of Grow's system.
Can both partners join from different locations?

Why is my rate for couples sessions different from my individual rate?
Reimbursement for couples and family therapy CPT codes is determined by each insurance plan and is typically set at a different rate than individual therapy codes. This reflects how payors categorize these service types industry-wide — many plans have different coverage structures for couples and family therapy compared to individual care, and some plans do not cover them at all.
This is an industry-wide payor structure, not a Grow-specific decision. Refer to your rate sheet for your specific rates by plan.
Is the rate for these sessions on my rate sheet per person or per session?
The rate shown on your rate sheet for CPT code 90847 (family or couples psychotherapy with the identified client present) is per session, not per person. You bill once for the session regardless of how many participants attend. The same applies to 90846 (family or couples psychotherapy without the identified client present).
What if I don't offer couples therapy but a couple books with me?
There is currently no system-level way to prevent this from happening. We recommend adding a note to your profile's Ideal Clients section to deter these bookings. If a couple has already booked and you cannot see them, use the in-portal referral process or cancel the appointment. See What to do if a couple books with you unexpectedly above.
Can I refer a client for couples therapy while keeping them as an individual client?
Yes. Use the referral flow in your portal and select "Continue care for [client name]" when prompted. This keeps the client on your panel while generating a couples therapy referral. See Referring clients for couples therapy while continuing individual care above.
