Learn how to manage recurring measures and client check-ins.

Measurement-informed care is standard practice at Grow Therapy. You can choose the measures that best fit your practice by configuring your settings, sending customized check-ins, and using client responses to inform your treatment plans.

Which measures are available in the Grow Therapy EHR?


Digital versions of the following measures are available in the Grow Therapy EHR.

The Adult ADHD Self-Report Scale (ASRS) is a brief self-report screening tool developed by the World Health Organization and Harvard Medical School to support clinicians in identifying adults who may be experiencing clinically significant symptoms of attention-deficit/hyperactivity disorder (ADHD).

This brief 3-item screening tool is used to identify clients who may be experiencing hazardous or harmful patterns of alcohol use. It focuses on consumption frequency and quantity over the past year.

This assessment is a valuable tool for screening substance misuse in adolescents aged 10 to 17.

This four-question assessment is a helpful tool for screening substance dependence.

This structured clinical tool assesses the presence, severity, and intensity of suicidal ideation and behavior in both adults and children. It helps providers identify clients who may need additional support and informs clinical decision-making around safety planning and next steps. The C-SSRS is available in adult and child versions.

This self-administered anxiety questionnaire assesses and evaluates your health status over the previous two weeks.

This self-administered questionnaire is used to screen, diagnose, monitor, and measure the severity of depression symptoms. At Grow Therapy, providers working with clients ages 18+ will use the PHQ-9.

PROMIS (Patient-Reported Outcomes Measurement Information System) Pediatric measures evaluate emotional and behavioral symptoms in children and adolescents. The Depression, Anxiety, and Anger subscales assess negative mood and affect, fear and worry, and irritability and aggression, respectively. All three use a T-score format, where 50 represents the population average.

This questionnaire was adapted from a series of quality-of-life (QOL) measures that capture how well a person functions in daily life.

S2BI (Screening to Brief Intervention)

This validated screener assesses the frequency of substance use across multiple substance categories. It is designed to quickly identify adolescents who may benefit from brief intervention or further evaluation.

This questionnaire was adapted from the Scale To Assess Therapeutic Relationships (STAR) and Brief Revised Working Alliance Inventory (BR-WAI) measures, standardized questionnaires that evaluate the quality of a client's relationship with their provider. This measurement is sent starting with the third appointment.

The WHO-5 is a screener used to help a client rate their perception of well-being. This information can be used by providers to identify interventions that may be helpful for clients to increase their sense of well-being.

A preview of each measure is available in your provider portal. To preview a measure:

  • While logged in to your provider portal, select your name in the bottom left > Settings.

  • Select Client check-in from the menu.

  • Select the icon to the right of the measure you'd like to view to open the measure in a new tab.

Payor-required measure cadence


Payors may have different measurement timing requirements that override any Grow-automated cadence. To view the most up-to-date list of payor requirements and measure cadence:

  • While logged in to your provider portal, select your name in the bottom left > Settings.

  • Select Client check-in from the menu.

  • Select "What are payor-required measures?"

  • A pop-up modal will list the payor, each required measure, and its timing.


Configure your default settings


To configure your measurement preferences:

  • Select your name in the bottom left and select Settings.

  • Under the "Appointments" section, select Client check-in.

  • Select or deselect the measures you'd like to send to your clients using the checkboxes. Under "recipients," you can choose to select Only payor-required.

  • Scroll down and select Save Changes.


Send a customized check-in


If you prefer to set your clients' screening cadence manually, you can customize your default settings and choose to send personalized check-ins.

Via your profile settings

To send a customized check-in from your profile settings:

  • Click here or select your name in the bottom left and select Settings.

  • Under the "Appointments" section, select Client check-in.

  • Select Send custom check-in. From the dropdown menu, select the desired client's name.

  • Indicate which forms you'd like the client to complete and select Send. The measures will be sent to the client immediately after confirmation.

Via the client's profile

To send a customized check-in from a client's profile:

  • Select Clients from the sidebar navigation.

  • Search for the desired client and select their name to access their chart.

  • Under the client's name, select the Treatment tab. Scroll down to locate the "Measures" card, then select the three-dot menu, as shown below.

  • Select Send custom check-in. Indicate which forms you'd like the client to complete and select Send. The measures will be sent to the client immediately after confirmation.

Complete measures during an appointment


If you'd like to complete measures with the client during an appointment, you can do so from the client's chart or directly in your Grow Telehealth meeting room. The client must have an upcoming or active appointment for this feature to be available.

To complete measures from the client's chart:

  • Select Clients from the sidebar navigation.

  • Search for the desired client, then select their name to open their chart.

  • Under the client's name, select the Treatment tab.

  • Scroll down to locate the "Measures" card.

🌟 The "Measures" card group measures completed or requested by date. A "Pending" status will be indicated for measurement bundles that have been sent but have not yet been completed by the client.

  • Select the specific bundle you'd like to complete. The pop-up menu allows you to toggle between the measures. Any measure with an "Incomplete" status allows you to select Fill out for client. Click this button to begin a measurement.

Each completed measure is stored by date and can be accessed via the client's profile. To access previous results:

  • Click here or select Clients from the sidebar navigation.

  • Search for the desired client, then select their name to open their chart.

  • Select the Treatment tab, located below the client's name. This tab will display a graph summarizing clients' progress, provided they regularly complete their measures. You can switch between graph and grid views and choose specific measures to engage with the results.

  • Alternatively, measures are bundled by completion date in the "Measures" card within the Treatment tab. Locate this card by scrolling to the bottom of the Treatment tab if necessary. Select the date you wish to review. A pop-up menu will display each completed measure along with its accompanying score(s).

To view completed measures with the client in session:

  • Join your Grow Therapy meeting room and admit the client.

  • Select the Client Information button from the telehealth toolbar.

  • From the Client information tab, locate the Treatment tab to review a client's measurement responses.
