Manage client forms and measures during appointments with Grow Telehealth

Learn how to manage client forms during an appointment using Grow Telehealth.

Grow Telehealth allows providers to meet with clients remotely while seamlessly integrating with their Grow Therapy provider portal to review and manage client information in real-time.

Review forms and questionnaires


Before an appointment

To view completed measures in your Grow Therapy meeting room before the client joins the waiting room:

  • Join your Grow Therapy meeting room via one of the entry points in your provider portal. Once you enter, an "Upcoming today" menu displays a chronological list of your appointments for the day. Select the desired appointment, and the pop-out menu will display the associated client's information and appointment management.

  • From the Client information tab, scroll down to the "Forms & questionnaires" section > select the form or measure you'd like to review.

During an appointment

To view completed forms and measures during an appointment:

  • Join your Grow Therapy meeting room and admit the client.

  • Select the Client Informationbutton from the telehealth toolbar. The pop-out sidebar will display the associated client's information and appointment options.

  • From the Client information tab, scroll down to the "Forms & questionnaires" section > select the form or measure you'd like to review.

Complete forms during an appointment


If your client has not completed the requested forms or measures before a session, you can complete them with the client without leaving the meeting room.

To complete the measures with the client while in session:

  • Join your Grow Therapy meeting room and admit the client.

  • Select the Client Informationbutton from the telehealth toolbar. The pop-out sidebar will display the associated client's information and appointment options.

  • From the Client information tab, scroll down to the "Measures and forms" section. Here, you will find forms and measures along with their status (complete or incomplete). Select the form you'd like to complete with the client, and a fillable version will appear in the pop-out menu.

Administer the Columbia-Suicide Severity Rating Scale (C-SSRS)


🛠 Feature notice: Starting December 12, 2024, all providers using Grow Telehealth as their video platform will be able to administer the Columbia-Suicide Severity Rating Scale (C-SSRS) during their sessions. If you are using a different video platform and wish to administer the C-SSRS, a downloadable PDF version will be available in the client chart within your provider portal.

By introducing the Columbia-Suicide Severity Rating Scale (C-SSRS) measure in session, providers can ensure they have all the necessary information to deliver the best care and effectively address client safety. This feature is available to therapists and prescribers, and there are two ways to complete and record your client's answers during a Grow Telehealth session.

Access the C-SSRS via notifications

If a client indicates a need for additional screening based on their measurement scores, you will receive a notification prompting you to consider administering the Columbia-Suicide Severity Rating Scale (C-SSRS). Select Administer C-SSRS from the notification. The screener will appear directly in the pop-out menu, allowing you to begin the screening process immediately.

Access the C-SSRS by sending a custom check-in

Even if a client's scores do not indicate the need for the C-SSRS, the tools within Telehealth allow you to use your best clinical judgment and administer the screener if necessary by sending a custom check-in. To start a custom check-in via Telehealth:

  • Select the client information icon from the Telehealth navigation menubar

  • Under "Measures & forms," select Measures.

  • Select Start custom check-in and ensure the C-SSRS box is checked along with any additional forms you'd like the client to complete before selecting Continue.
