Learn how Grow's Care Coordination program assists you in supporting clients who require higher levels of care (HLOC).

The Grow Therapy Care Coordination program helps clients access higher levels of care (HLOC) based on their provider’s recommendations. The team works closely with providers and clients to facilitate referrals to Grow’s direct HLOC partners, assist with insurance navigation when needed, and identify programs that align with the provider’s referral. Throughout this process, Care Coordination serves as the main point of contact, keeping both the provider and client informed, offering guidance, and supporting a smooth transition to the recommended program.

⚠️ Note: Grow Therapy's Care Coordination program is specific to intensive outpatient programs (IOP) and specialty treatment programming.


Key program terms


Higher Levels of Care (HLOC) - HLOC programs are designed for clients needing more intensive support than an outpatient setting can provide. These programs offer a more structured environment with increased supervision to better meet the client’s needs.

Examples of these programs include Intensive Outpatient Programs (IOPs), Partial Hospitalization Programs (PHPs), residential treatment, and inpatient care.

Intensive Outpatient Program (IOP) – IOPs offer a higher level of support than standard outpatient care. They provide structured treatment sessions, three to four hours daily, several times weekly. These programs are designed for individuals who need more intensive treatment but do not require inpatient care. They allow individuals to develop coping skills while continuing with their daily responsibilities.

Medication-Assisted Treatment (MAT) - A treatment for substance use disorders that combines medications with counseling. These medications help manage withdrawal symptoms, reduce cravings, and support recovery, while therapy addresses the mental and emotional aspects of addiction.

Specialty Programs – A treatment program focused on specific conditions, such as eating disorders or substance abuse.

Partial Hospitalization Programs (PHP) – A more intensive program than IOP. PHPs can range between 4 to 8 hours a day, 3 to 5 days a week, depending on the client's needs, but not requiring overnight stays.

Direct Referral Partners – Vetted, high-quality treatment providers or organizations that Grow has a partnership for directly sending referrals.

What types of higher levels of care (HLOC) does Grow’s Care Coordination program support?

Grow assists with referrals for Intensive Outpatient Programs (IOPs) and specialty mental health programs, including eating disorder treatment.

Grow's Care Coordination program does not coordinate referrals for:

  • Inpatient psychiatric hospitalization

  • Partial Hospitalization Programs (PHP)

  • Clients in active crisis. Each provider is responsible for managing clients in crisis or who may need psychiatric hospitalization.

Please note that requests for in-person care or psychological testing services are not considered HLOC and are not facilitated by the Grow Care Coordination team.

In some cases, insurance providers require clients to go through specific pathways to obtain access to an HLOC program. When this happens, the Grow’s Care Coordination team assists by guiding the client through the required steps for their insurance. If a direct referral partner isn’t a fit, we help connect clients with alternative programs that align with provider recommendations.


Submit a HLOC referral request


How do providers submit a Care Coordination request?

Providers can submit a Higher Level of Care (HLOC) referral directly in their provider portal via the Help Widget. To streamline the process, please have the following client information available:

  • Client's full name and date of birth

  • Client's contact information (phone and email)

  • Insurance provider and type of HLOC needed (e.g., IOP, eating disorder treatment)

  • Preferred referral partner (if applicable)

  • Reason for referral

  • Whether the client will return to provider care post-discharge

To submit an HLOC referral request in your provider portal:

  • Log into your Provider portal.

  • Select the Help Widget in the bottom right.

  • Select the "Send us a message" option.

  • Input "Higher Level of Care Referral" into the message and send.

  • Follow the prompts to submit the required information. The information will be sent directly to the Care Coordination team, and you will typically receive a response within one business day.

Who is eligible for Care Coordination services?

Care Coordination at Grow is available to any client who has completed at least one session with their provider and is clinically appropriate for a specialty program or Intensive Outpatient Program (IOP). This support is only accessible to clients who have an established relationship with a Grow provider and whose provider has submitted a Care Coordination request via the Help Widget. Clients cannot self-refer to higher levels of care (HLOC) or Care Coordination support.

When is the Care Coordination team available?

Care Coordination at Grow operates Monday through Friday, 8 AM—5 PM PST. Requests are typically addressed within 1 business day. If a request is submitted outside of business hours, it will be reviewed the next business day.

How long does connecting a client with an IOP or specialty program take?

The Care Coordination team works to connect clients to HLOC programs as quickly as possible. Clients are typically connected to a partner within 24-48 hours, though in some cases, it may take up to 5 days

Who should providers contact for questions after submitting a request?

Providers should follow up within the original request thread in the provider portal or the Help Widget in the lower right-hand corner for specific request updates. For general Care Coordination inquiries, providers can email care@growtherapy.com.
