Treatment plans are fundamental to the collaborative experience between clinician and client. Clearly defining and documenting your treatment plan's objectives, interventions, and timeframes enables you to monitor progress toward the established goal(s) and ongoing outcomes. Grow allows you to update a client's treatment plan through progress notes and status updates, indicating different levels of progress and accomplishments throughout a client's time in your care.

All Medicaid clients can view treatment plans in their client portal, which is a Medicaid requirement. Grow's product team will make treatment plan visibility available to all therapist and client portals in February 2025.
Establish a treatment plan
Each client's treatment plan is created based on an initial assessment or progress note. Once you have completed the initial treatment plan, this information will automatically appear in all future notes. You can revise the treatment plan or indicate that all information is up to date using any follow-up template.
Preview a treatment plan
Treatment plans can only be updated when a progress note is written. However, you can preview each treatment plan from the client's chart without opening notes tied to an individual appointment.
To access a treatment plan via the client's chart:
Navigate to Clients via the sidebar.
Search and select the client's name to bring up their chart.
Ensure the Treatment tab is selected beneath the client's name. From here, select the treatment plan you wish to preview.
The pop-up preview defaults to "Current goals," which shows all treatment goals currently in progress. To review previous or completed treatment plans, select the Past or All Goals tabs.
Update a treatment plan
Treatment plans are living documents. Regularly reviewing and updating goals supports focused treatment. Treatment plans can only be updated using a follow-up note template. To update a treatment plan:
To begin the documentation process, initiate an invoice. From the note template options, select Progress Note or Soap Note from the pop-up menu.
Complete the required fields as you proceed through each section of the note. Alternatively, select Treatment Plan in the side navigation.
Select "Reviewed and updated as described below" to update the fields of a treatment plan as needed. The changes will be reflected in the client's chart.
Share a treatment plan

You can share the treatment plan(s) with your client while completing the documentation for the latest appointment. After the 24-hour editing window has closed, the treatment plan will be sent to the client.
To share a treatment plan with your client:
Begin the documentation process by initiating an invoice.
Establish, update, or complete a treatment plan in the last section of your notes.
In the "Share treatment plan" section, toggle your sharing preference to 'ON.' After the 24-hour edit period ends, your client can view their treatment plan in their portal.
To check the sharing status of a treatment plan:
Navigate to Clients via the sidebar menu.
Search and select the client's name to open their chart.
Navigate to the Treatment tab beneath the client's name.
The "Treatment plan" section displays the sharing status of the treatment plan.
Preview the client experience
Once you establish a treatment plan and share it with your client, they will be prompted to view it the next time they log in to their client portal. Sharing a treatment plan is optional unless the client has Medicaid.
Updated treatment plans
Once a treatment plan is updated, a status indicating this will be available in the client's view of their treatment plans.
Completed treatment plans
Clients will receive notifications as you update the treatment plan status in your follow-up documentation. These notifications focus on which goal within the treatment plan has been marked with a new status.
