Your client portal keeps a record of every form and mental health check-in you've completed with Grow Therapy. Whether you want to revisit a past assessment or review how your responses have changed over time, you can access everything from the Forms tab in your account.

Access completed check-ins
To access your completed forms and mental health check-in history:
Log in to the client portal.
Select Account from the left sidebar menu.
Select the Forms tab.
Select the γ»γ»γ» ellipses next to the form or check-in you want to review.
If you select a check-in, a dialog box will appear with a summary of symptom changes since your last check-in at the top.
Scroll down to review your detailed responses and see how your scores were calculated.

What's the difference between a form and a check-in? Forms are documents like intake paperwork completed at the start of care.
