Review completed forms & check-ins

Learn how to access your completed forms in the client portal.

As you progress in your therapy journey, it's essential to reflect on how far you've come. Your client portal makes it easy to review your mental health check-in history whenever you need to revisit previous assessments. This guide explains locating your completed forms and check-ins in the client portal.


Access completed check-ins


To access your completed forms and mental health check-in history:

  • Log in to the client portal.

  • Select Account from the left sidebar menu.

  • Select the Forms tab.

  • Click View next to the form or check-in you want to review.

  • If you select a check-in, the results will appear in a dialog box, with a summary of symptom changes since your last check-in displayed at the top. Scroll down to review your detailed responses and see how your scores were calculated.

  • If you select a form or transcription, a digital copy will appear in a dialog box.

🌟 TIP: Be sure to complete all forms! Incomplete forms will be marked with a yellow "Requires completion" note. Filling them out helps improve the accuracy of your progress data.
