Before and during your therapy sessions, your provider may ask you to fill out a short intake form and periodic questionnaires. These help your provider understand your needs and allow you to track your progress over time. This guide explains how to find and complete these forms in the client portal.
⚠️ Important: You'll need to activate your client portal account before you can access forms or tasks. If you haven't yet set up your password, follow the steps in Activate your client portal before continuing.

Complete the intake form
If your provider has enabled intake forms, you'll receive an email with a link to complete yours after booking your first appointment. If possible, complete this form before your intake session so your provider can review your information ahead of time. If you have questions about the form, you can ask your provider during your session.
✉️ Access the intake form via email
After booking a full-length appointment, you'll receive an email reminder with a direct link to your intake form. If multiple forms are required, they'll appear together with a progress bar at the top showing your completion status.
🌟 Note: If you didn't receive the email, check your spam folder. Your pending tasks also appear in your portal dashboard under My tasks — see the steps below.
🖥️ Access the intake form in your client portal
To complete the intake form in your client portal:
Log in to your client portal and navigate to your Dashboard.
Under "My tasks," select Complete intake paperwork for (provider name).
Complete the required fields of the intake form, then select Submit in the top right to send the form to your provider for review.
🌟 Tip: If "My tasks" is empty or your intake isn't appearing, first confirm your account is activated. If it is and forms still aren't showing, try clearing your browser cookies and cache — refer to Clear your cookies and cache for steps. If forms still don't appear, contact our support team.
Complete mental health check-ins
While the intake form is a one-time form per provider, you'll receive shorter questionnaires periodically as part of your ongoing care. These help your provider track your progress and adjust your treatment over time. You can complete them via email or your client portal.
If your provider uses Grow Therapy's telehealth platform, GAD-7 (Generalized Anxiety Disorder 7-item scale) and PHQ-9 (Patient Health Questionnaire-9) check-ins can also be completed directly in the telehealth room before or during your session.

You'll receive an email reminder with a link to your check-in forms. If you're assigned multiple forms, they'll appear together with a progress bar at the top.

To complete check-ins in your client portal:
Log in to your Client Portal and navigate to your Dashboard.
Under "My tasks," select Prepare for your appointment.
A new tab will open where you can complete your forms. If multiple forms are assigned, they'll be grouped together with a progress bar at the top.
⏳In the Grow Therapy Telehealth room
If your provider uses Grow Therapy's telehealth platform, your GAD-7 and PHQ-9 check-ins can be completed directly in the telehealth room — before or during your session.
Log in to your client portal and ensure Dashboard is selected in the sidebar menu.
Select Join session to enter the telehealth waiting room. If you have incomplete GAD-7 or PHQ-9 check-ins, they'll open automatically in a pop-out panel on the right side of the screen.
Complete the forms at any time before your provider joins, or during the session itself.
Track your progress
Your check-in results are available to view immediately after you submit. You can also review your full history at any time under "My progress" on your dashboard.
Scores are calculated from your responses to each form. For a full explanation of how different questionnaire scores are calculated, refer to Understanding intake forms and mental health check-ins.
Keep in mind that scores represent a snapshot in time — it's normal for them to fluctuate. If you have questions about your results or how your provider uses them, ask your provider directly. They can explain how this information shapes your care and what to expect going forward.
