Log in to your client portal and navigate to your Dashboard.
Under "My tasks," select Complete intake paperwork for (provider name).
Complete the required fields of the intake form, then select Submit in the top right to send the form to your provider for review.

While the intake form is only completed once per provider (if required), you'll receive measurement-based questionnaires periodically as part of your ongoing "mental health check-ins." You can access these check-ins through email, your client portal, or by entering your provider's Grow Therapy Telehealth room at least five minutes before your appointment.
