Learn how to complete intake forms and recurring mental health check-ins.

Once you schedule an intake appointment, your provider may send you an intake form and measurement-based questionnaires. These questionnaires help your provider understand the best approach for your care and allow you to monitor your progress over time. Each provider has their own methods, preferences, and areas of expertise, so your experience will be customized to meet your needs. If you have any questions about these forms, please ask your provider how they will be utilized in your care.


Complete the intake form


If your provider has sent you an intake form, you can access it using one of the methods listed below. If possible, it’s essential to complete this form before your intake session so your provider can prepare adequately for your initial assessment. If you need assistance completing the form, don’t hesitate to ask your provider for help during your session.

Access the intake form via email

If your provider has chosen to automatically send intake forms and questionnaires, you will receive an email reminder with a direct link to complete them after you book a full-length appointment. If multiple forms are required, they will be grouped together, and a progress bar will appear at the top to indicate your completion status.

Access the intake form in your client portal

Your client portal is an excellent resource for checking any outstanding tasks related to your care journey. In the "My Tasks" section of your Dashboard, you will find a list of any incomplete forms, questionnaires, or other actions that need your attention. If you need to activate your client portal, please refer to this guide.

To complete the intake form in your Client Portal:

  • Log in to your Client Portal and navigate to your Dashboard.

  • Under "My tasks," select Complete intake paperwork for (provider name).

  • Complete the required fields of the intake form and select Submit in the top right to send the form to your provider for review.


Complete mental health check-ins


While the intake form is completed once for each provider that requires one, you'll receive measurement-based forms periodically as a "mental health check-in." Access these check-ins through email, your Client Portal, or by entering your provider's Grow Therapy Telehealth room at least five minutes before your appointment.

You can access the link to complete measurement-based forms directly from your reminder email. If multiple forms are required, they will be grouped together, and a progress bar will appear at the top to indicate your completion status.

To complete measures in your Client Portal:

  • Log in to your Client Portal and navigate to your Dashboard.

  • Under "My tasks," select Prepare for your appointment.

  • A new tab will open to access and complete the forms. If multiple forms are requested, they will be grouped together, and a progress bar will be visible at the top to indicate your completion status.

If your provider uses Grow Therapy's Telehealth for sessions, you can complete your measurement-based forms in the waiting room by arriving more than five minutes before your appointment.

  • Log in to your Client Portal and ensure Dashboard is selected in the sidebar menu.

  • Select Join session to access the session waiting room. If you have incomplete measures, you'll see a prompt asking you to complete your mental health check-in.

  • Select Start check-in. This activates the questionnaires in the right-side pop-out panel, including a status bar indicating the number of forms to complete. If you are admitted into the meeting room before completing the forms, your progress will be saved, and the form will be carried over to your session.

Measure your progress


We encourage you to discuss your questionnaire results with your provider during your next appointment to better understand what they might mean for you. Your provider can offer personalized advice and explore potential treatment options.

Please keep in mind that these results represent a snapshot in time, and it's normal for these levels to fluctuate.

Understand how scores are calculated

Responses to each measurement-based questionnaire are scored in points, which are then summed to produce a total score. Expand the drop-down options below to preview each scoring system.

Depression

Depression scores are calculated using your answers from the PHQ-9 questionnaire and are scored using the following point system:

Therapeutic Relationship

This questionnaire was adapted from the Scale To Assess Therapeutic Relationships (STAR) and Brief Revised Working Alliance Inventory (BR-WAI) measures, standardized questionnaires that evaluate the relationship quality between a client and their provider. The results are measured with the following point system:

Track your progress

Each time you complete a new mental health check-in, you can view your results and monitor progress made on previous submissions immediately afterward.

When you complete measures regularly, you can monitor your progress at any time in your Client Portal. To view your results in your Client Portal:

  • Log in to your Client Portal.

  • Select Account from the sidebar navigation.

  • Select the Forms tab from the top navigation. Your check-ins are organized by completion date. Select View to review your results for the selected date.
